Female members aged 50+ (who have not had a mammogram in the last 2 years), or where a mammogram has been declared clinically appropriate, can book a mammogram appointment via I-MED (https://i-med.com.au/book-appointment) before 31 December 2021 and AIA Vitality will cover the $250 cost of a bi-lateral screen mammogram via cashback into your bank account.

HOW TO CLAIM YOUR CASHBACK:

1. Attend your appointment and pay $250 (must be paid in full by member on day of appointment)
2. Send copy of tax invoice to: queries@aiavitality.com.au before 31 January 2022 with the subject line: BREAST SCREEN CASHBACK and include your AIA Vitality membership number in the body of the email.  

Cashback will be paid within 30 days from receipt of email. It is your responsibility to ensure cashback bank account details are correct to avoid processing delays. 

Please note that members must have a referral from GP or specialist and appointments must be booked with I-MED via this link: https://i-med.com.au/book-appointment. AIA Vitality membership number must be included in booking. All COVID-19 safety protocols must be followed as instructed.
